Intestinal retransplantation.

چکیده
ADVANCEMENT in surgical technique, perioperative care, and the availability of effective immunosuppressive agents has made intestinal transplantation a feasible procedure in patients with intestinal failure. Despite improvements in survival, however, the procedure still carries a higher patient and graft loss when compared to other organs. Re-transplantation (re-Tx) in this patient population may be necessary in cases of primary graft failure. We present our experience with 6 patients who underwent re-Tx of intestinal allografts and discuss possible determinants of survival.

Basic considerations for the procurement of intestinal grafts.
From May 1990 to September 1993.59 patients received intestinal transplantation at our center. Twenty-two required an isolated intestinal (small bowel (S8]) graft. 26 required a combined liver and intestinal (SBIL) graft. and II required a multiviscerai (MV) graft. Three additional grafts were obtained for S8 (n = I). S81L (n = I). and MV (n '" I) retransplantation. AU 62 grafts were obtained f...

Under FK 506-based immunosuppression, the entire cadaver small bowel except for a few proximal and distal centimeters was translated to 17 randomly matched patients, of whom two had antigraft cytotoxic antibodies (positive cross-match). Eight patients received the intestine only, eight had intestine in continuity with the liver, and one received a full multivisceral graft that included the live...
متن کاملLiver retransplantation for adult recipients
Living donor liver graft can be used for the first or second liver transplantation. The timing of retransplantation also should be stratified as 2 types according to the reoperation timing. Combination of these two classifications results in 6 types of living donor liver transplantation (LDLT)-associated retransplantation. However, late retransplantation to LDLT might have not been performed in...
متن کاملHeart retransplantation.
Retransplants comprise only a small minority (3-4%) of heart transplants, however outcome following retransplantation is compromised. Risk factors for a poor outcome following retransplantation include retransplantation early (<6 months) after primary transplant, retransplantation for acute rejection or early allograft failure, and retransplantation in an earlier era. The incidence of rejection...
متن کاملIs lung retransplantation indicated? Report on four patients.
As more lung transplantations are performed, many patients will suffer graft failure and will be considered for retransplantation. This article reviews the case management reports of four patients who received lung or heart/lung retransplantation, with overall disappointing results. The pros and cons of lung retransplantation are discussed.
